Recombinant human insulin is produced using genetically engineered Escherichia coli or Saccharomyces cerevisiae that express the human insulin gene. The process involves fermentation, cell lysis, inclusion body solubilization, refolding, enzymatic modification, and multi-step chromatographic purification to produce pharmaceutical-grade insulin.

E. coli expressing proinsulin genes are fermented in large bioreactors. Inclusion bodies containing proinsulin are isolated, solubilized in urea, and the protein is refolded by controlled oxidation. Trypsin and carboxypeptidase B cleave the C-peptide to yield mature insulin, which is purified by reversed-phase and ion-exchange chromatography.
