Metformin is the most widely prescribed oral antidiabetic drug and the first-line treatment for type 2 diabetes globally. The synthesis involves the reaction of dimethylamine hydrochloride with dicyandiamide (cyanoguanidine) at elevated temperatures. Despite being known since 1922, metformin's full mechanism of action is still being elucidated. Its low cost, proven safety profile, and potential benefits beyond diabetes (cancer prevention, anti-aging) make it one of the most important drugs in current medicine.

Dimethylamine hydrochloride is heated with dicyandiamide in toluene or xylene at 130-150 degrees C for several hours. The metformin hydrochloride product precipitates from solution and is collected by filtration, washed, and recrystallized from methanol-isopropanol to achieve pharmaceutical-grade purity.

Environmental Impact
The synthesis is relatively clean with high atom economy. Toluene solvent is recycled. The main environmental concern is dimethylamine emissions, which are controlled by scrubbing systems. Wastewater contains low levels of organic nitrogen compounds requiring biological treatment.
